Pathogenesis of Endometriosis: New Insights into Prospective Therapies
This review examines the mechanisms driving endometriosis pathogenesis, including cell proliferation and metastasis, to inform the development of novel therapeutic strategies beyond hormonal treatments.

This review paper discusses endometriosis pathogenesis and prospective therapeutic targets, focusing on how crosstalk among inflammatory and immune mechanisms contributes to ectopic lesion initiation, adhesion, proliferation, invasion, angiogenesis, and pain/infertility. It synthesizes evidence on multifactorial causes such as altered immunity and estrogen-dependent processes, highlighting roles for macrophages, dendritic and natural killer cells, and inflammatory mediators including MIF and MCP-1, as well as the proinflammatory transcription factor NF-κB that regulates cytokines and survival pathways. It also summarizes findings of elevated cytokines and growth factors (e.g., IL-1, IL-6, IL-8, COX-2, VEGF, TNF-α) in eutopic/ectopic tissues and peritoneal fluid, and notes that pharmacologic inhibitors of NF-κB and cytokine antagonists (e.g., tocilizumab) are discussed as potential therapies. As a narrative review, it does not present new experimental data and relies on previously published studies, without specifying selection or systematic methods. Relevance to endometriosis: this paper is centrally about endometriosis, providing a mechanistic overview of inflammatory/immune pathways and therapeutic targets linked to endometriosis progression and symptoms.
