Intrafollicular inflammatory cytokines but not steroid hormone concentrations are increased in naturally matured follicles of women with proven endometriosis

This matched case-control study compared follicular fluid and serum inflammatory cytokine concentrations in 17 women with laparoscopically and histologically confirmed endometriosis (rAFS II–IV) undergoing natural cycle IVF with 17 age-matched women without diagnosed endometriosis, with samples collected at follicle aspiration and cytokines/hormones measured by immunoassay. In follicular fluid, IL-1β and IL-6 were significantly higher in the endometriosis group (and IL-1β/IL-6 showed a tendency with disease severity), while detectable interleukins were higher in follicular fluid than serum; estradiol in follicular fluid was lower in severe endometriosis, and follicular IL-1β/IL-6 were not correlated with intrafollicular estradiol. A key limitation is the small sample size and the cross-sectional association with outcomes not directly assessed in this analysis beyond contextual interpretation. This paper is centrally about endometriosis — it identifies upregulated intrafollicular inflammatory cytokines (IL-1β, IL-6) in naturally matured follicles from women with proven endometriosis.
