This case-control study compared pre-laparoscopy serum interleukin-6 (IL-6) and CA125 in 76 women with laparoscopically confirmed endometriosis and 76 women with normal laparoscopic findings, with endometriosis stratified as minimal-to-mild versus moderate-to-severe. Women with recent inflammatory disease, elevated ESR, fever, prior endometriosis medications, or other key confounders were excluded, and IL-6 and CA125 were measured by ELISA and immunoassay, respectively, before surgery. Both IL-6 and CA125 were higher in the endometriosis group, but ROC analysis showed low diagnostic performance for IL-6 (AUC 0.54) and CA125 (AUC 0.69), and the authors report that IL-6 was elevated mainly in moderate-to-severe disease, not minimal-to-mild.
