Interleukin-6 as A Useful Predictor of Endometriosis-Associated Infertility: A Systematic Review
This systematic review of 15 studies found that elevated IL-6 concentrations in serum and peritoneal fluid correlate with endometriosis-associated infertility, suggesting IL-6 as a potential biomarker.

This systematic review searched Medline, Embase, Cochrane systematic reviews, and ClinicalTrials.gov (through December 2022) for English-language prospective studies and randomized controlled trials measuring interleukin-6 (IL-6) in serum, peritoneal fluid, follicular fluid, or endometrial biopsy specimens from women with endometriosis-associated infertility and comparing them with relevant control groups. Across 15 included studies, the authors reported higher serum and peritoneal fluid IL-6 levels in endometriosis-associated infertility versus controls, with one study also finding a correlation between peritoneal fluid IL-6 and an American Fertility Society scoring metric, while evidence for differences in follicular fluid and endometrial specimen IL-6 was inconsistent or null. Limitations explicitly noted include heterogeneity of control groups, potential influence of sample size, concurrent inflammatory conditions, hormone/therapy use, assay sensitivity and possible detection of nonfunctional or inhibited IL-6, and dependence of IL-6 on menstrual cycle phase, endometriosis stage, and lesion type. This paper is centrally about endometriosis — it systematically reviews IL-6 levels as predictors of endometriosis-associated infertility.
