Molecular Characteristics of the Endometrium in Uterine Adenomyosis and Its Biochemical Microenvironment
This review summarizes molecular characteristics of the endometrium and biochemical microenvironments in uterine adenomyosis, exploring potential roles of endometrial and myometrial contributions to its pathogenesis.

This review synthesizes in vitro and in vivo studies using human and mouse models to summarize molecular characteristics of adenomyosis-associated eutopic endometrium within its biochemical microenvironment, focusing on differential genetic expression/epigenetic modulation and estrogen-induced epithelial-mesenchymal transition. It also highlights evidence that the myometrium may contribute via smooth muscle metaplasia and fibroblast-to-myofibroblast transdifferentiation driven by local biochemical factors. A key limitation is that the literature remains unclear on whether adenomyosis is primarily genetically determined versus microenvironmentally induced, and it is uncertain whether endometrial molecular characteristics remain stable across reproductive life. This paper is centrally about endometriosis-like uterine adenomyosis — it specifically reviews molecular and biochemical microenvironment changes in adenomyosis endometrium.
