This review summarizes evidence for progesterone resistance in endometriosis, focusing on studies of women with endometriosis using tissue and gene/protein expression analyses of eutopic endometrium and endometriotic lesions, as well as mechanistic findings tied to progesterone receptor signaling and downstream progesterone-responsive pathways. It reports that progestins are effective for many patients but fail in about one-third of symptomatic women, with evidence including altered or reduced PGR (notably loss/attenuation of PR-B), lower PGR levels in nonresponders, and attenuated expression of multiple progesterone-regulated genes and proteins in endometriosis, even when progesterone exposure is controlled in vitro. Proposed mechanisms include disrupted PR-A/PR-B balance, impaired target gene networks relevant to decidualization and endometrial receptivity, and broader effects of progestins on immune-inflammatory, angiogenic, apoptotic, and extracellular-matrix/invasion pathways.
