Complications and outcomes of pregnant women with adenomyosis in Japan
This study found that larger adenomyosis size and diffuse type are associated with adverse pregnancy outcomes, including increased miscarriage, preterm delivery, and fetal growth restriction rates in Japanese women.

This nationwide, multicenter retrospective survey in Japan evaluated clinical records from 262 pregnant women with adenomyosis (excluding cases with uterine myoma or endometriosis) to assess pregnancy complications and how they relate to adenomyosis size, type (focal vs diffuse), and pretreatment before pregnancy. Women were diagnosed by gynecologists using ultrasonography and/or MRI, and complications were tracked across outcomes including miscarriage (overall and after 12 weeks), preterm delivery, fetal growth restriction, PPROM, placental disorders, and other events; the authors report that pregnancy complications occurred in 72.1% overall and, except for uterine infection being more common in the medication group, complication rates did not differ significantly by pretreatment. Larger adenomyosis size was associated with higher rates of miscarriage after 12 weeks and cervical incompetency, while diffuse-type adenomyosis showed higher rates of pre-eclampsia/PIH and uterine infection compared with focal type; a key limitation is the retrospective design and reliance on facility-reported imaging-based diagnosis with incomplete data (10 cases excluded).
