Current Non-Invasive Imaging Techniques Used in the Diagnosis of Adenomyosis
This review discusses non-invasive imaging techniques for adenomyosis, finding that 2D ultrasound with 3D ultrasound or elastography offers similar diagnostic performance to MRI, which remains reliable despite cost and time limitations.

This paper is a literature review (PubMed search from 1998 to August 2022) evaluating non-invasive imaging methods for diagnosing adenomyosis, mainly using studies that compared imaging to postoperative histology, and secondarily those comparing against MRI. It finds that conventional transvaginal 2D ultrasound has historically high sensitivity but limited specificity, while applying Morphological Uterus Sonographic Assessment (MUSA) terminology improves conventional ultrasound diagnostic outcomes; however, distinguishing adenomyosis from concomitant benign uterine conditions remains challenging, especially when lesions have subtle ultrasound features and/or require experienced examiners. Adding 3D ultrasound or elastosonography to 2D ultrasound increases diagnostic sensitivity, and qualitative elastography may help differentiate benign uterine pathologies based on lesion stiffness, while MRI has higher diagnostic specificity and sensitivity but is limited by cost and longer examination time. This paper is centrally about adenomyosis imaging—reviewing non-invasive ultrasound (2D/3D, elastography) and MRI diagnostic accuracy and the role of consensus terminology (including MUSA) for reporting adenomyosis-related lesions.
