Hormonal Therapy in Endometriosis and Adenomyosis: Progestins

Progestins are synthetic compounds that mimic the effects of progesterone. For over 50 years, oral progestins have been demonstrated to be effective in the treatment of endometriosis, and symptom improvement reported between 60% and 94%. According to the fact that both endometriosis and adenomyosis share similar origins regarding estrogen dependency, medical treatment strategies follow the same principles which aim to reduce the endogenous estrogen production or inducing endometrial differentiation with progestins. Progestins are available in many forms, including oral preparations (dienogest, norethindrone acetate, dydrogesterone, medroxyprogesterone acetate, etc.), injections, subdermal implants, and intrauterine systems. Continuous progestin use is an effective therapy for the treatment of painful symptoms associated with endometriosis, but there has been no evidence of progestin use being superior to other types of treatment in endometriosis-related pain symptoms. On the other hand, the intrauterine system (LNG-IUD) seems to be the most promising treatment option for the medical treatment of adenomyosis.
