Efficacy of Combined Levonorgestrel-Releasing Intrauterine System with Gonadotropin-Releasing Hormone Analog for the Treatment of Adenomyosis
Combining gonadotropin-releasing hormone analog (GnRHa) treatment with levonorgestrel-releasing intrauterine system (LNG-IUS) implantation significantly reduced menstrual flow, dysmenorrhea, and uterine volume in adenomyosis patients.

This study evaluated a combined regimen of gonadotropin-releasing hormone analog (goserelin GnRHa) followed by re-implantation of a levonorgestrel-releasing intrauterine system (LNG-IUS/Mirena) in 21 women with adenomyosis and enlarged uteruses (>12 weeks’ gestational size), many of whom had prior Mirena expulsion. After 3–4 cycles of GnRHa, uterine volume and uterine length decreased to enable Mirena placement; among those who retained the device, menstrual flow and dysmenorrhea decreased significantly by 3–12 months, while uterine size remained below pre-GnRHa levels though uterine volume rose relative to the immediate post-GnRHa measurements at 6–12 months. Irregular bleeding was common, and LNG-IUS expulsion occurred in 3/21 patients (14.3%), with recurrence of dysmenorrhea in expulsed cases; the authors also noted limitations including small sample size, no controlled design, and short follow-up.
