Efficacy of levonorgestrel releasing intrauterine system in the treatment of symptomatic adenomyosis: comparison with dienogest

This randomized controlled trial (April 2022–March 2023) compared the effectiveness of a levonorgestrel-releasing intrauterine system (LNG-IUS) versus oral dienogest in 32 women aged 25–45 with symptomatic adenomyosis (menorrhagia and dysmenorrhea), assessing outcomes over 3 and 6 months. The LNG-IUS group had significantly greater reductions in menstrual pain measured by VAS at both 3 months and 6 months, and more women had regular menstrual flow at follow-ups, whereas heavy bleeding persisted in a larger proportion of the dienogest group. Hemoglobin improved more in the LNG-IUS group by 6 months, and although adverse effects were more frequently reported with dienogest, the difference was not statistically significant; the study’s key limitation is the small sample size.
