Norethindrone Acetate in the Medical Management of Adenomyosis

This retrospective chart review at a university-based hospital evaluated norethindrone acetate (NA) as medical management for 28 premenopausal women (ages 27–49) with moderate-to-severe dysmenorrhea and menorrhagia diagnosed with adenomyosis using clinical features plus transvaginal ultrasound, with MRI confirmation and endometrial biopsy for women over 35. The study found marked symptom improvement, with significant reductions in dysmenorrhea and bleeding scores by 3 months that were maintained during treatment, and only 21.4% reported short-duration breakthrough bleeding, with none reported after two months under a “three-weeks-on and one-week-off” regimen. The authors’ main limitations were the small sample size, relatively short follow-up, lack of objective measures for uterine bleeding, no standardized quantification beyond symptom scoring, and no follow-up after discontinuation.
