Diagnosis and Surgical Management of Adenomyosis

Adenomyosis is the infiltration of endometrial tissue into the myometrium. The benign condition can remain asymptomatic and undiagnosed or present in those with pain, bleeding, and/or subfertility. While there are a number of proposed mechanisms described in this chapter, the exact pathophysiology is yet to be elucidated. Diagnosis, previously reserved for definitive surgical pathology, has become more widespread given improved non-invasive imaging techniques including MRI and ultrasound; and as such, dedicated research into the potential deleterious reproductive effects of adenomyosis has expanded in the recent decades. This chapter reviews the array of fertility-sparing surgical management options for diffuse adenomyosis and localized adenomyoma and explores the impact on future fertility.
