Cytokines, Angiogenesis, and Extracellular Matrix Degradation are Augmented by Oxidative Stress in Endometriosis

This nested case-control retrospective study measured serum levels of multiple cytokines and mediators of angiogenesis and extracellular matrix (ECM) degradation, along with peripheral blood mononuclear cell NF-κB activation, in women with surgically confirmed endometriosis (N=80) versus tubal factor infertility controls (N=80). Using ELISA and flow cytometry, the authors found that several pro-inflammatory cytokines, VEGF, MMP-2 and MMP-9, and COX-2 were significantly higher in endometriosis, while TIMP-1 and TIMP-2 were significantly lower; NF-κB expression also increased. Multivariate analyses identified IL-10 as the most significant variable discriminating endometriosis from controls, and the paper proposes an interplay model where oxidative stress–dysregulated NF-κB affects cytokines, leading to IL-10–linked activation of MMPs and subsequent ECM degradation and angiogenesis. The authors’ main caveat is the retrospective design and reliance on serum/PBMC markers rather than direct mechanistic measurements in the lesions themselves.
