Zusammenfassung Sowohl die Pathogenese als auch die Ätiologie der Endometriose sind bis zur heutigen Zeit nicht vollkommen geklärt. Ein Konzept, das in den letzten Jahren viel an Bedeutung gewonnen hat, sieht die Endometriose als chronische Entzündungserkrankung. Die Endometrioseläsionen stellen einen permanenten inflammatorischen Prozess dar, der sich selbst erhält und verstärkt. Als Folge dieser Erkenntnisse zur Pathogenese eröffnen sich neue Perspektiven bei Diagnostik und Therapie der Erkrankung. Bis heute gilt die Endometriose als unheilbar. Die aktuelle Therapie beruht auf Suppression der Östrogensynthese und greift damit gravierend in den Hormonhaushalt ein. Die Inhibition der inflammatorischen Prozesse ermöglicht eventuell neue Therapieansätze mit spezifischen Behandlungszielen, ohne während des therapeutischen Zeitraums die Reproduktionsfähigkeit der Frau einzuschränken. Abstract Our understanding of the aetiology and pathophysiology of endometriosis remains unclear. A concept, which has been established over the last few years, proposes that endometriosis is a chronic inflammatory disease. Endometriotic lesions present an inflammatory process that amplifies and maintains itself permanently. The inflammation accounts for the pathogenesis of endometriosis. This concept allows new perspectives for diagnostic tools as well as targeted therapy of this disease. To date, endometriosis remains incurable. The present therapy is based on the suppression of estrogen synthesis and intervention on the basis of hormonal regulation. Inhibition of inflammatory processes might serve as a new, targeted drug treatment without limiting the reproductive possibilities of a woman during the treatment phase.
