Risk factors associated with endometriosis: importance of study population for characterizing disease in the ENDO Study

The ENDO Study used a prospective matched design to study environmental and lifestyle determinants of endometriosis, enrolling 495 currently menstruating women aged 18–44 undergoing laparoscopy/laparotomy (operative cohort) and 131 currently menstruating women without prior surgical endometriosis (population cohort), with standardized interviews, anthropometrics, and biospecimen collection. Endometriosis diagnosis was based on surgical visualization (with rASRM staging) in the operative cohort and pelvic MRI findings in the population cohort, and analyses across the two cohorts identified risk factors associated with diagnosis. The only consistent risk factor across both cohorts was a history of infertility, with adjusted odds ratios of 2.43 in the operative cohort and 7.91 in the population cohort; several other associations appeared only in one cohort and sensitivity analyses assessed robustness when restricting to histologically confirmed or stage 3–4 disease and using alternative comparison groups. A key limitation emphasized is that MRI cannot reliably detect minimal/mild endometriosis, complicating incidence estimates. This paper is centrally about endometriosis — it characterizes study-population design and identifies infertility as a consistent risk factor across surgically and MRI-detected cohorts.
