This paper reviews the historical evolution and current understanding of endometriosis diagnosis, treatment, and prevention, contrasting earlier theories based on retrograde menstruation and implantation with newer genetic–epigenetic (G–E) and related concepts that allow cumulative molecular “incidents” across cell divisions, influenced by oxidative stress and the peritoneal microbiome. The authors argue that lesion-by-lesion clonal differences, variable cancer-driver mutations, and heterogeneity in progesterone resistance and aromatase activity are more consistent with cells acquiring irreversible genetic/epigenetic changes (and possibly inducing reversible metaplasia) than with a single implantation mechanism; they also note limitations such as incomplete knowledge about transgenerational epigenetic transmission and specific documented epigenetic incidents. They further describe how the peritoneal microenvironment, low-grade inflammation and fibrosis may constrain growth (self-limiting lesion severity), while microbiota alterations and hormonal milieu may contribute to initiation and variable symptom timing and diagnostic delay. This paper is centrally about endometriosis — it presents and develops the G–E theory and its implications for diagnosis, treatment, natural history, and prevention in endometriosis.
