A Promise in the Treatment of Endometriosis: An Observational Cohort Study on Ovarian Endometrioma Reduction by N-Acetylcysteine
This observational cohort study found that N-acetylcysteine treatment in women with ovarian endometriosis resulted in a slight cyst diameter reduction compared to untreated controls, with fewer cysts increasing in size and more patients canceling surgery due to cyst decrease or disappearance.

This observational cohort study compared ultrasound-measured changes in ovarian endometrioma mean diameter and volume over a 3-month waiting-list interval between Italian patients who accepted oral N-acetylcysteine (NAC) and those who did not, with outcomes assessed early in the follicular phase by a blinded experienced sonographer. NAC treatment was associated with preventing the significant increase in cyst size seen in controls, with fewer enlarging endometriomas and more cysts showing size reduction (including disappeared cysts), while reported pain scores (VAS for dysmenorrhea, dyspareunia, chronic pelvic pain) were also lower in many treated patients; the authors report no adverse reactions and similar rASRM staging and pathology among those who proceeded to laparoscopy. The main caveats include nonrandomized allocation based on acceptance/refusal of NAC and a relatively short follow-up limited to the pre-surgical interval.
