Aged garlic extract as a potential prophylactic to reduce the progression of endometriosis and associated pain burden
This perspective article proposes that aged garlic extract, due to its immunomodulatory and antioxidant properties, could be used prophylactically to reduce endometriosis progression and associated pain.

This perspective article reviews evidence on endometriosis pathophysiology—particularly inflammation, oxidative stress, angiogenesis, and neurogenic pain—and proposes that aged garlic extract (AGE), an ethanolic garlic-bulb extract processed for 12–20 months and enriched in organosulfur compounds such as S-allyl-cysteine and S-allylmercaptocysteine, could serve as a prophylactic to attenuate endometrial progression and pain. It draws on prior human and preclinical findings, including a trial where garlic powder reduced endometriosis pain symptoms, in vitro studies showing reduced adhesion/proliferation markers in activated endometrial stromal cells with aged black garlic extract, and mechanistic links between antioxidants/anti-inflammatory effects and pain reduction. The paper’s major limitation is that it is hypothesis-driven and does not present original clinical or experimental AGE data, relying on synthesized literature.
