Influential factors on sexual function in infertile women with endometriosis: a path analysis

This cross-sectional study recruited 220 infertile women aged 18–45 years with laparoscopically confirmed endometriosis from an infertility clinic in Tehran and used validated Persian questionnaires to assess sexual function (FSFI), anxiety and depression (HADS), sleep quality (PSQI), and pain intensity (VAS for pelvic pain and dyspareunia), alongside BMI, education, physical activity, and endometriosis stage. Path analysis found that anxiety, depression, sleep quality, BMI, level of education, endometriosis stage, and dyspareunia had direct effects on sexual function, while sleep quality, anxiety, pelvic pain, and depression were major mediators contributing to decreased sexual function. The paper also reports indirect effects whereby lower physical activity and higher BMI worsened sexual function through anxiety and sleep quality, and that anxiety related to poorer sleep quality and depression. A key limitation is the cross-sectional design, so causal directions cannot be established beyond the model tested, and the authors excluded 11 participants due to incomplete data.
