Endometriosis-Associated Ovarian Carcinomas: How PI3K/AKT/mTOR Pathway Affects Their Pathogenesis
This review summarizes current knowledge on PI3K/AKT/mTOR pathway dysregulation in endometriosis-associated ovarian clear cell and endometrioid carcinomas, highlighting its role in their pathogenesis and interactions with other signaling molecules.

This paper is a narrative review discussing how the PI3K/AKT/mTOR pathway may contribute to the initiation and malignant progression of endometriosis-associated ovarian cancers, focusing on mTOR complex biology (mTORC1/mTORC2) and downstream effects on growth, translation, apoptosis, and autophagy, alongside upstream regulation by PTEN and growth factor signaling. The review synthesizes evidence that mTOR signaling is involved in endometrial homeostasis and that endometriosis and its precursor lesions show increased PI3K/AKT/mTOR activity, including studies reporting higher phosphorylated AKT in eutopic and ectopic tissues, PTEN loss, and PI3K/AKT/mTOR-related genetic alterations in endometriosis-associated settings, with supporting mouse data where PTEN deletion or AKT inhibition alters lesion establishment. A key limitation is that, as a review, it does not present new experiments or a systematic analysis to quantify effect sizes or resolve mechanistic conflicts across all cited studies. This paper is centrally about endometriosis-associated ovarian carcinomas — specifically how PI3K/AKT/mTOR pathway signaling may drive the transition from endometriosis to endometriosis-associated ovarian cancer.
