Increased Circulating MMP-2 Levels in Infertile Patients With Moderate and Severe Pelvic Endometriosis
This study found no differences in follicular fluid MMP-2 or MMP-9 between infertile patients with and without endometriosis, but identified significantly higher serum MMP-2 in those with advanced stages.

This cross-sectional study compared MMP-2 and MMP-9 levels in follicular fluid and serum MMP-2 levels among 30 infertile women undergoing IVF: stage I/II endometriosis, stage III/IV endometriosis, and controls with infertility due to tubal and/or male factors. MMP concentrations were measured by zymography, with serum collected in the early follicular phase and follicular fluid collected on the day of oocyte retrieval; the study also assessed whether endometriosis severity altered these levels. The authors found no differences in follicular MMP-2 or MMP-9 levels between groups, but serum MMP-2 was significantly higher in infertile women with advanced (stage III/IV) endometriosis than in those with milder (stage I/II) disease. The paper’s limitation, as implied by its design, is its small sample size and lack of longitudinal or functional assessment beyond measured biomarker levels. This paper is centrally about endometriosis — it links advanced pelvic endometriosis severity in infertile IVF patients to increased circulating (serum) MMP-2 while reporting no follicular MMP-2 or MMP-9 differences.
