Medical management of endometriosis
This review summarizes recent evidence on hormonal and nonhormonal medical management options for pelvic pain associated with endometriosis, highlighting first- and second-line treatments.

This paper is a literature review of medical management options for pelvic pain associated with endometriosis, focusing on higher-quality evidence from clinical trials (including RCTs and evidence from trial registries) published between December 2021 and March 2024, and summarizing outcomes related to pain and quality of life. It finds that first-line hormonal suppressive therapies such as combined oral contraceptives and progestogens improve endometriosis-associated symptoms in evidence syntheses and observational studies, while limitations include relatively sparse prospective data for some widely used treatments, dated or limited RCTs for others, and risks of bias (e.g., attrition in long-term LNG-IUD follow-up) plus trial designs excluding treatment-resistant patients. Dienogest is highlighted as a well-studied progestogen with RCT evidence showing improvements in nonmenstrual pelvic pain, and newer GnRH antagonism (relugolix with add-back) demonstrates efficacy over placebo in phase III trials with additional safety signals including small bone mineral density changes in follow-up. Relevance to endometriosis: the review synthesizes medical options centrally for endometriosis and also discusses adenomyosis only in the context of a COCP observational study including women with deep infiltrating endometriosis with or without coexisting adenomyosis.
