Polypoid endometriosis: a mimic of malignancy
This case report describes polypoid endometriosis, a rare variant mimicking malignancy on imaging, which decreased in size after hormone therapy discontinuation.

This paper describes a single case of a 60-year-old postmenopausal woman with an incidentally discovered polypoid pelvic mass centered in the cul-de-sac, abutting a cervical remnant anteriorly and the rectum posteriorly, with normal CA-125 and no malignancy symptoms. MRI showed a lobulated heterogeneous T2-hyperintense mass with a peripheral low T2 rim, no restricted diffusion on DWI/ADC, and only mild heterogeneous enhancement, while FDG-PET/CT demonstrated only mild uptake; after an initial inconclusive colonoscopic biopsy, transvaginal biopsy found endometrioid glands with squamous metaplasia embedded in endometrial stroma that was CD10-positive, consistent with polypoid endometriosis. The authors acknowledge that imaging and FDG uptake were non-specific and that definitive diagnosis required histopathology, with limited literature largely based on case reports/series.
