This clinicopathologic study analyzed 24 cases of polypoid endometriosis in patients aged 23–78 years and reviewed the literature, focusing on its clinical, gross, and microscopic features and distinction from Müllerian neoplasms. Lesions occurred most often in the colon, ovary, and uterine serosa, frequently presented as pelvic or vaginal masses or bowel obstruction, and showed endometriotic glands and stroma with varied hyperplasia, metaplasia, hemorrhage, fibrosis, and thick-walled vessels; absence of stromal atypia and characteristic periglandular changes generally distinguished them from adenosarcoma. Follow-up was available for only 17 patients, most of whom had no residual disease, and the authors noted possible associations with exogenous hormones or hyperestrogenism and rare premalignant or neoplastic evolution. This paper is centrally about endometriosis — the rare polypoid variant and its differentiation from Müllerian adenosarcoma.
