Adnexal mass with extremely high levels of CA-125 and CA19-9 but normal Human Epididymis Protein 4 (HE4) and Risk of Ovarian Malignancy Algorithm (ROMA): Endometriosis or ovarian malignancy? A case report

This case report describes an 18-year-old virgin with 2 weeks of constant low-grade lower abdominal pain and imaging evidence of a large complex right ovarian adnexal cyst with mild free fluid; tumor marker testing showed extremely high CA-125 (6484 IU/mL) and CA19-9 (1309 IU/mL) while HE4 and ROMA were low-risk and other tumor markers were normal. Despite the low malignancy risk suggested by HE4/ROMA, laparotomy was performed and intraoperatively the omentum contained extensive black puckered endometriotic lesions, with frozen section and histology confirming endometriosis involving the omentum and an endometrioma; CA-125 and CA19-9 decreased rapidly after surgery. The paper’s main limitation is that it is a single patient report, so the diagnostic implication for other patients with similar marker patterns cannot be generalized. Relevance to endometriosis: it specifically demonstrates that advanced omental endometriosis/endometrioma can produce CA-125 and CA19-9 elevations that mimic ovarian malignancy, while HE4 and ROMA remain low, making it directly about endometriosis.
