HE4, Ca125 and ROMA algorithm for differential diagnosis between benign gynaecological diseases and ovarian cancer

This study evaluated the diagnostic performance of serum HE4, Ca125, and the ROMA algorithm to distinguish primary ovarian cancer from benign gynecological diseases, using measurements from 119 women with benign conditions, 29 with primary ovarian cancer, 32 with ovarian cancer receiving chemotherapy (including 18 with progressive disease), 6 post-treatment disease-free patients, and 32 healthy controls. HE4 and Ca125 were elevated in cancer patients, while HE4 was not elevated in endometriosis; ROMA also showed higher values in cancer, with AUCs of 0.92 (HE4), 0.911 (Ca125), and 0.945 (ROMA), and higher overall sensitivity/specificity for ROMA compared with Ca125 and with HE4. The limitation explicitly noted by the design is that benign cases were heterogeneous and the study size—particularly for cancer subgroups such as disease-free patients—was relatively small.
