Oxidative Stress and Antioxidant Defense in Endometriosis and Its Malignant Transformation
This review of literature from 1990-2014 found that benign endometriosis involves excess reactive oxygen species and oxidative stress, while endometriosis-associated ovarian cancer may be linked to enhanced antioxidant defense mechanisms.

This paper reviews the literature on how oxidative stress and antioxidant defenses contribute to endometriosis and its rare malignant transformation, with emphasis on hemoglobin, heme, and iron/redox imbalance after repeated pelvic bleeding and hemolysis. Using a MEDLINE-based selection of studies published between 1990 and 2014 (and excluding articles due to potential selection/detection/reporting bias), it integrates mechanisms by which released heme and labile iron promote ROS generation via autoxidation and Fenton/Haber-Weiss chemistry while antioxidant systems (e.g., SOD, GPX, catalase, NFE2L2/NRF2-regulated detoxification) counterbalance oxidant stress; it also discusses pathways such as HO-1 induction and NRF2-mediated cytoprotection, plus the paradox that iron-rich conditions may increase mutagenesis and drive carcinogenesis. A major limitation is that the work is an integrative narrative review without new experimental data, relying on heterogeneous, selectively identified publications with acknowledged bias risks. This paper is centrally about endometriosis — specifically, how redox balance involving oxidative stress, antioxidant defenses, and hemoglobin/heme/iron mechanisms relates to endometriosis and its malignant transformation (endometriosis-associated ovarian cancer).
