New insights on the pathogenesis of endometriosis and novel non-surgical therapies
This review discusses current understanding of endometriosis pathogenesis and evaluates novel non-surgical treatments, including dienogest, aromatase inhibitors, GnRH antagonists, anti-TNF-α, SERMs, SPRMs, and HIFU, assessing their efficacy and potential.

This paper reviews prevailing and emerging models of endometriosis pathogenesis—such as retrograde menstruation, metaplasia, stem-cell differentiation, immune dysregulation, inflammatory cytokine/angiogenic signaling, and estrogen–progesterone receptor imbalance—and links these mechanisms to potential non-surgical therapies. It summarizes high-level evidence for several pharmacologic approaches, including dienogest (progestin effects that reduce aromatase/COX-2/PGE2 gene/protein expression and alter ER/PR ratios), aromatase inhibitors, GnRH antagonists (with elagolix-associated reductions in gonadotropins/estradiol and improvements in pelvic pain and quality of life), anti–TNF-α strategies, and emerging SERMs/SPRMs such as bazedoxifen and asoprisnil (highlighting lesion-size or pathway effects plus safety and efficacy limitations). A stated caveat is that many of these novel therapies are not fully tested in humans or remain in early-phase clinical trials, and the overall evidence varies by agent and outcome. This paper is centrally about endometriosis — it synthesizes pathophysiology and discusses novel non-surgical therapeutic mechanisms and evidence for treating endometriosis.
