Reduction in Endometrioma Size with Three Months of Aromatase Inhibition and Progestin Add-Back
A 3-month treatment of aromatase inhibition with progestin add-back significantly reduced mean endometrioma diameter by 50% and volume by 75% in women with endometriosis.

This pilot study enrolled eight consecutive premenopausal women (14 ovarian endometriomas) to receive an off-label 3-month course of letrozole 5 mg daily plus norethindrone acetate 5 mg add-back, with pre- and posttreatment transvaginal ultrasound measurements and pain scores from the Biberoglu and Behrman scale. The authors reported that mean endometrioma diameter decreased by about 50% (4.6 cm to 2.3 cm) and that mean endometrioma volume decreased by about 75% (60.1 cm³ to 15.0 cm³), alongside reductions in dyspareunia, dyspareunia/nonmenstrual pelvic pain, and overall pain scores. A major limitation is the very small, non-randomized, pilot design with a short follow-up window, and the authors explicitly note the need for larger double-blind, placebo-controlled randomized trials. This paper is centrally about endometriosis — it evaluates aromatase inhibition with progestin add-back to reduce ovarian endometrioma size and associated symptoms in premenopausal women.
