Is body mass index associated with the incidence of endometriosis and the severity of dysmenorrhoea: a case–control study in China?
While overall body mass index was not associated with endometriosis incidence, obesity increased endometriosis risk and was a risk factor for severe dysmenorrhea in Chinese women.

This retrospective case–control study in China investigated whether body mass index (BMI) is associated with the incidence of surgically confirmed endometriosis and whether BMI relates to the severity of dysmenorrhoea among women with endometriosis. It included 709 women with endometriosis (diagnosed January 2018–August 2019) and 807 age-matched controls without a history of suggestive endometriosis, using hospital electronic records to compare BMI categories (WHO Asian/Chinese cutoffs) and dysmenorrhoea severity assessed by VAS (last 2 years). Overall, BMI was not associated with endometriosis incidence, but obesity (BMI >27.5 kg/m²) was associated with higher odds of endometriosis compared with normal weight, and among women who reported dysmenorrhoea, obesity was linked to greater odds of severe dysmenorrhoea. A key limitation explicitly implied by the design is that controls were selected based on self-reported lack of suggestive endometriosis, so misclassification may affect associations. This paper is centrally about endometriosis — it tests whether BMI and dysmenorrhoea severity are linked with endometriosis incidence in Chinese women.
