Impact of endometriosis on quality of life and work productivity: a multicenter study across ten countries

This multicenter cross-sectional GSWH study recruited 1,418 premenopausal women aged 18–45 across 16 hospitals in 10 countries scheduled for laparoscopy for suspected endometriosis-associated pelvic pain or subfertility, or for sterilization, excluding those with prior surgical diagnosis. In the week before surgery, participants completed questionnaires assessing health-related quality of life (SF36v2) and work productivity/activities (WPAI:GH), and endometriosis was diagnosed visually at laparoscopy and staged using rAFS; the study also measured diagnostic delay from reported care pathways. Endometriosis was found in 52.5% of participants, and affected women reported poorer HRQoL (especially bodily pain and certain physical/mental role limitations) and greater work productivity loss (10.8 vs 8.4 hours/week versus symptomatic controls), with diagnostic delay associated with reduced physical HRQoL in affected women. The paper’s limitations include its cross-sectional design and reliance on patient-reported recall for diagnostic delay, and it uses visual-only laparoscopic diagnosis, and found heterogeneity by center and funding context—factors that may reflect more than biological severity.
