Association Between Endometriosis and Hypercholesterolemia or Hypertension
Laparoscopically confirmed endometriosis was prospectively associated with increased risk of hypercholesterolemia and hypertension, and conversely, these conditions were associated with a higher risk of endometriosis.

This prospective cohort study in the Nurses’ Health Study II examined whether laparoscopically-confirmed endometriosis predicts incident physician-diagnosed hypercholesterolemia and hypertension, and whether the reverse association exists, using repeated biennial questionnaire data and multivariable Cox models with marginal structural models to address time-dependent confounding. Women with laparoscopically-confirmed endometriosis had higher risks of hypercholesterolemia (RR=1.25, 95% CI 1.21–1.30) and hypertension (RR=1.14, 95% CI 1.09–1.18), and the associations were unchanged with MSMs; conversely, hypercholesterolemia (RR=1.22) and hypertension (RR=1.29) were associated with higher risk of laparoscopically-confirmed endometriosis. A major limitation acknowledged is potential outcome misclassification from self-reported diagnoses, addressed partially via prior validation and by censoring endometriosis reports without laparoscopic confirmation when studying endometriosis as an exposure. This paper is centrally about endometriosis — it quantifies bidirectional associations between laparoscopically-confirmed endometriosis and hypercholesterolemia and hypertension in a large prospective cohort.
