The recurrence rate of ovarian endometrioma in women aged 40–49 years and impact of hormonal treatment after conservative surgery

This retrospective single-center study evaluated recurrence rates and risk factors for ovarian endometrioma after conservative surgery in 408 women aged 40–49, with ≥6 months follow-up using ultrasound-based criteria for recurrence. Among participants, 8.3% (34/408) experienced recurrence, with a median time to recurrence of 22.4 months and cumulative recurrence rates of 3.7% at 12 months, 6.7% at 24 months, 11.1% at 36 months, and 16.7% at 60 months. Recurrence was associated with multilocular cysts, prior surgery for ovarian endometrioma, and prior salpingectomy, while in multivariate analysis only large cyst size (>5.5 cm) remained a risk factor; the authors report that post-operative hormonal medication (types and duration) did not reduce recurrence.
