Combination of non-invasive and semi-invasive tests for diagnosis of minimal to mild endometriosis
Combining serum IL-6 measurements with the presence of endometrial nerve fibers enhanced diagnostic sensitivity and specificity for minimal-mild endometriosis compared to single tests.

This prospective study evaluated whether a non-surgical biomarker (serum interleukin-6 measured from blood drawn the day before laparoscopy) combined with a semi-invasive histologic feature (endometrial biopsy immunohistochemistry for pan-neuronal marker protein gene product 9.5–positive nerve fibres in the functional layer) could diagnose minimal–mild endometriosis. Among 114 women undergoing laparoscopy for infertility and/or pelvic pain, 40 had no pathologic findings and 74 had endometriosis confirmed by histopathology and divided into minimal–mild (stages 1–2) versus moderate–severe (stages 3–4). Serum IL-6 at a 15.4 pg/ml threshold yielded 89.5% sensitivity and 82.5% specificity for minimal–mild disease, while nerve fibres alone showed 92% sensitivity and 80% specificity; combining the two tests increased sensitivity to 100% and specificity to 92.5%. This paper explicitly relates to endometriosis by focusing on improved diagnostic accuracy for minimal–mild endometriosis using IL-6 plus endometrial nerve-fibre detection.
