Umbilical Endometriosis: A Systematic Literature Review and Pathogenic Theory Proposal
This systematic review of 232 umbilical endometriosis cases found it comprises about 20% of abdominal wall lesions, primarily occurs as a primary form, and presents with pain, catamenial symptoms, and bleeding, possibly originating from implanted regurgitated endometrial cells.

This systematic literature review (PRISMA; PROSPERO-registered) assessed 25 eligible retrospective and prospective studies/case series reporting umbilical endometriosis (UE) lesions confirmed histopathologically, using PubMed and Embase searches (1950–2021) and extracting epidemiologic, clinical, and recurrence data; study quality was screened with MINORS (excluding scores <6). Across 232 UE cases, UE accounted for about 21% of abdominal wall endometriosis cases in studies reporting totals, with most lesions classified as primary (~68%) and pain and catamenial-associated symptoms reported in roughly 83% of cases. Approximately one third of women with UE had a prior endometriosis diagnosis, concomitant pelvic endometriosis was seen in about 35% when assessed, and postoperative recurrence was low (7/148, ~4.7%) though long-term evidence is limited and most studies had small sample sizes and were largely English-language. This paper is centrally about endometriosis — it specifically reviews umbilical endometriosis (Villar’s nodule) and proposes a pathogenic theory based on epidemiology, symptoms, and lesion distribution.
