This paper reports a rare case of spontaneous umbilical endometriosis (Villar’s nodule) in a 29-year-old nulligravid woman with a painless, occasionally bleeding umbilical lesion and no prior abdominal surgery/trauma and no classic endometriosis symptoms. The lesion was excised and histopathology showed endometrial-type glands rimmed by endometrial-type stroma, confirming primary cutaneous endometriosis, while pelvic examination and transvaginal ultrasound found no pelvic endometriosis. A key limitation is that, as a single case report, it cannot establish prevalence or causal mechanism, and the patient’s broader disease status was assessed with imaging rather than definitive surgical mapping.
