Using ultrasonography, MRI, fine needle aspiration biopsy, and PET-CT to evaluate a suspected urachal cancer, the workup showed a small solitary umbilical nodule with atypical cells and hemosiderin-laden macrophages; adenocarcinoma was initially suspected by cytology, and malignancy could not be definitively excluded. The authors performed surgical excision of the rigid umbilical lesion with the umbilicus and surrounding tissues, and histopathology identified endometriosis with endometrial glands without malignancy, with no recurrence at 1 year.
