This case report describes the successful joint treatment of a 36-year-old woman with de novo umbilical endometriosis using a collaborative approach between minimally invasive gynecologic surgery and plastic surgery. The surgical team performed a full excision of the endometrial implant down to the anterior rectus fascia, followed by reconstruction using a novel four-flap umbilicoplasty technique to ensure optimal cosmetic outcomes and preserve supraumbilical hooding. Pathology confirmed the presence of endometrial tissue in both the umbilical mass and pelvic structures, and postoperative follow-up indicated excellent healing with no recurrence of cyclic bleeding.
