This paper reports a case of primary (spontaneous) umbilical endometriosis in a 24-year-old nulliparous woman with a slowly enlarging, menstruation-associated bleeding umbilical nodule, despite no clinical or ultrasound evidence of pelvic endometriosis. The authors suspected umbilical endometriosis based on symptoms and imaging, excised the 1 cm nodule deep to the fascia with a rim of normal tissue under local anesthesia, and confirmed the diagnosis histologically and by immunohistochemistry showing ER/PR positivity and stromal CD10 expression. Postoperative recovery was uneventful, and after 24 months of follow-up there were no signs of local relapse or other clinical or ultrasonographic disease. The paper emphasizes that because there are no standardized guidelines for umbilical endometriosis, management remains based on rarity and limited follow-up data, and laparoscopic exploration to exclude occult pelvic disease was not performed in an asymptomatic patient.
