This chapter reviews primary umbilical endometriosis, focusing on ectopic endometrial tissue in the umbilicus that presents as a painful, discolored umbilical nodule, and highlights the diagnostic challenge of distinguishing it from other causes of an umbilical nodule. It notes that imaging lacks pathognomonic features, making diagnosis difficult, and that hormone treatment may temporarily reduce symptoms but symptoms can reoccur after discontinuation. The chapter states that surgical exploration and excision are safe and definitive for this condition, with the main caveat being the lack of specific imaging criteria. This paper is centrally about endometriosis — it specifically addresses primary umbilical endometriosis and its diagnosis and management.

Abstract

Endometriosis is a common benign disease, which is defined by the presence of endometrial tissue outside the uterus. Primary umbilical endometriosis is a rare disorder, caused by the presence of ectopic endometrial tissue in the umbilicus, which can present as a painful, discolored umbilical nodule. The clinical distinction between umbilical endometriosis and other causes of an umbilical nodule is difficult. Imaging modalities do not show any pathognomonic signs for umbilical endometriosis, which will make the diagnosis even harder to recognize. Treatment with hormones may diminish the symptoms of umbilical endometriosis temporarily, but after discontinuation of the hormonal therapy, they may reoccur. Surgical exploration and excision is a safe and definitive treatment of this condition.
