This cross-sectional study (Oct 2009–Oct 2010) compared eight oxidative stress markers measured in serum on the day of oocyte retrieval and in follicular fluid of infertile women with and without endometriosis undergoing controlled ovarian stimulation for ICSI, assessing how well they predicted clinical pregnancy and live birth using ROC analyses. Among analyzed samples, women with endometriosis showed a distinct oxidative stress pattern: higher serum GSH and superoxide dismutase, lower total antioxidant capacity, and higher follicular 8-hydroxy-2-deoxyguanosine and vitamin E, with several serum-versus-follicular fluid differences also reported within the endometriosis group. For prediction, no serum marker in the endometriosis group adequately predicted clinical pregnancy or live birth, while follicular 8-hydroxy-2-deoxyguanosine was an adequate predictor of clinical pregnancy; in the non-endometriosis group, 8-hydroxy-2-deoxyguanosine predicted clinical pregnancy and live birth in serum and glutathione predicted both outcomes in follicular fluid. A key limitation explicitly reflected by the design is the cross-sectional measurement at one time point with reduced paired sampling after exclusions, yielding uneven sample sizes for predictive analyses. This paper is centrally about endometriosis — it evaluates oxidative stress markers in serum and follicular fluid from women with endometriosis-related infertility and tests their ability to predict clinical pregnancy and live birth after ICSI.
