This paper describes the WERF EPHect Phenome and biobanking harmonization project’s development of standardized, consensus evidence-based clinical and covariate phenotype collection procedures, focusing on the endometriosis patient questionnaire (EPQ) and its associated tiered data-collection approach (minimum required vs standard recommended). Using workshops with international endometriosis research centers, literature searches, expert review, and patient/volunteer piloting, the authors define which questionnaire domains to include (e.g., pelvic pain, subfertility/reproductive history, menstrual history and hormone use, medical/surgical history, and medications), and specify validated measures for pain intensity, pain affect, and pain catastrophizing, while noting that some life-course exposures were intentionally omitted unless needed for study goals. They explicitly state limitations that the EPQ was not designed to capture many potentially etiologic exposures beyond what was considered crucial for broad patient characterization, and that resource/logistical differences across centers motivated the use of two collection tiers.
