Abstract

Objectifs : Evaluer les connaissances des médecins généralistes dans la prise en charge de l endométriose, dans les conditions du déclin démographique des gynécologues médicaux. Matériel et méthodes : Cent médecins généralistes du département de la Seine Maritime, région de la Haute Normandie, ont participé à une enquête concernant le diagnostic et la prise en charge thérapeutique de l endométriose. La sélection des médecins a été réalisée en respectant les critères de l appartenance géographique et de la parité. Résultats : Parmi les médecins généralistes interrogés (MGI), 44% effectuent plus d une consultation gynécologique par semaine. Plus des deux tiers des médecins généralistes se considèrent peu ou pas du tout à l aise dans la prise en charge de l endométriose. Ils estiment la prévalence de l endométriose à 5-6%. Plus de la moitié des MGI ne peuvent pas citer 3 des symptômes cardinaux de l endométriose. Seulement 38% des MGI pratiquent un examen clinique orienté en cas de suspicion d endométriose. L IRM n est pratiquée en première intention que dans 28% des cas. L avis spécialisé est pris d emblé par 24% des MGI et 57% après bilan et/ou test thérapeutique, et seulement 52% sollicitent cet avis auprès du Centre Hospitalier de Rouen. Concernant le traitement médical, 64% des MGI sont d accord que l aménorrhée constitue la base du traitement. La participation à des formations gynécologiques au cours de derniers 5 ans et le suivi de plus de 3 femmes traitées pour une endométriose ont été des facteurs associés à la probabilité de répondre correctement aux questions. Conclusion : Les médecins généralistes ne semblent pas suffisamment sensibilisés à la problématique du diagnostic et de la prise en charge de l endométriose, ce qui contribue probablement au retard diagnostic caractéristique à cette pathologie chronique. La participation à la formation continue en gynécologie et les échanges entre les spécialistes et les médecins qui suivent les patientes déjà traitées pour endométriose semblent améliorer la qualité des réponses au questionnaire.
