Cross-sectional imaging showed a partly cystic, partly solid/hemorrhagic lesion with subtle rim enhancement, no pancreatic duct dilation, and no visible duct communication; definitive diagnosis was ultimately made by histopathology, which demonstrated multiple cysts lined by epithelium without dysplasia surrounded by ovarian-type stroma with smooth muscle cells and extensive hemorrhage, supporting endometriosis externa involving the pancreas. Postoperatively she had a short respiratory complication that resolved and remained asymptomatic at 5-year follow-up, with a small residual cystic lesion considered postoperative.
