Clinical Predictors of Long-term Success in Ultrasound-guided High-intensity Focused Ultrasound Ablation Treatment for Adenomyosis
This study found that higher nonperfused volume ratios and older age predicted long-term success in ultrasound-guided high-intensity focused ultrasound ablation for adenomyosis, while lower BMI and higher acoustic power were associated with reduced recurrence risk.

This retrospective study evaluated long-term outcomes (median follow-up 40 months) of ultrasound-guided high-intensity focused ultrasound (USgHIFU) ablation in 208 premenopausal women with MRI-confirmed adenomyosis and severe dysmenorrhea and/or menorrhagia, aiming to identify clinical success and symptom-relapse predictors. Using contrast-enhanced ultrasound before and immediately after treatment, the authors quantified nonperfused volume (NPV) of lesions and found that higher NPV ratio predicted clinical success in multivariate analysis, with an NPV ratio cutoff of 39.2% (89.9% success for ≥39.2% vs 62.0% for <39.2%); age and treatment-related parameters further refined associations. Dysmenorrhea recurred in 26% of patients (median 12 months), and multivariate recurrence modeling identified BMI as a risk factor and mean acoustic intensity as a protective factor. A key limitation is that outcomes were based on phone interviews and dysmenorrhea VAS, and 22 participants were excluded due to being unreachable, which may introduce bias.
