Diagnosing adenomyosis with MRI: a prospective study revisiting the junctional zone thickness cutoff of 12 mm as a diagnostic marker
This study found that while junctional zone thickness cutoff of 12 mm on MRI had limited diagnostic accuracy for adenomyosis, irregular junctional zone appearance showed good diagnostic accuracy in premenopausal women.

This single-center prospective observational study enrolled 93 premenopausal women with benign gynecological conditions who underwent blinded MRI assessment and subsequent hysterectomy with histopathological confirmation, to evaluate whether junctional zone (JZ) thickness (JZmax) ≥12 mm and specific JZ morphological features could diagnose adenomyosis. Adenomyosis was confirmed in 61% of participants, but JZmax showed no significant association with diagnosis (AUC 0.57, p=0.26) and did not differ between groups, and the ≥12 mm cutoff performed similarly with no significant difference. In contrast, irregular JZ appearance was most associated with adenomyosis, showing sensitivity 74% and specificity 83% (p<0.001).
