Ultrasound-guided percutaneous microwave ablation for adenomyosis: efficacy of treatment and effect on ovarian function

This prospective study enrolled 142 premenopausal women with symptomatic adenomyosis who underwent ultrasound-guided percutaneous microwave ablation and were followed with serum estradiol (E2), FSH, prolactin (PRL), and CA125 measured before treatment and at 3, 6, 9, and 12 months, along with uterine volume assessments. The authors found no significant changes in E2 or FSH levels across follow-up and reported no menopausal transition symptoms among participants; they also noted that one case of amenorrhea after ablation was attributed to Asherman’s syndrome with hormone levels remaining within the normal range. In contrast, mean CA125 and PRL levels were significantly decreased at multiple follow-up timepoints, and changes in CA125 correlated with uterine volume (not PRL). A major limitation was attrition in follow-up (numbers fell at later timepoints), and the study was not designed to assess long-term fertility or hormonal dynamics beyond 12 months.
