The Expression and Functionality of Transient Receptor Potential Vanilloid 1 in Ovarian Endometriomas

This study examined TRPV1-positive nerve fibers and TRPV1 expression/functionality in ectopic endometrium from women with ovarian endometriomas versus endometrium from women without endometriosis, and correlated TRPV1-positive nerve fiber density with dysmenorrhea severity. Using immunohistochemistry and functional testing in ectopic endometrial stromal cells (EESCs), the authors found higher TRPV1-positive nerve fiber density in ectopic implants than in controls, with a positive correlation to dysmenorrhea severity. TRPV1 expression in EESCs was elevated after stimulation with inflammatory mediators including PGE2 and TNF-α, and TRPV1 activation induced release of nitric oxide and IL-1β. A key limitation is that the paper primarily links TRPV1-related findings to pain by correlation and does not directly establish causality for hyperalgesia. This paper is centrally about endometriosis—specifically TRPV1 expression and nerve–stromal cell functionality in ovarian endometriomas and its association with dysmenorrhea.
