Management of Adenomyosis in Subfertile Women and Pregnancy Outcome
Combined conservative surgery and Gn-RHa therapy improved pregnancy rates and delivery prospects in subfertile women with adenomyosis compared to Gn-RHa alone.

This retrospective study assessed pregnancy and symptom outcomes in 40 subfertile women with pathology-proven symptomatic uterine adenomyosis managed over 8 years, comparing Gn-RH agonist (goserelin) therapy alone (n=22) versus conservative cytoreductive surgery followed by Gn-RH agonist (n=18). After 6 courses of Gn-RH agonist, patients were followed for 6–36 months, and outcomes included symptom relief, clinical pregnancy, successful deliveries, and CA-125 changes (including post-surgery reduction in the combined-surgery group). Women in the surgery+Gn-RHa group had significantly higher pregnancy and term delivery rates than the Gn-RHa-only group, with pregnancies extending up to 3 years after completion in the surgery group; CA-125 decreased after surgery, and symptom improvement occurred in both groups, although some women later required hysterectomy. The authors note a key limitation that, due to the study’s nature, a well-conducted randomized trial is needed to better assess comparative benefits.
